How does our appointment system work?

You will be contacted in the month of your birth to be seen for any long term conditions you may be diagnosed with. 

You may be contacted earlier if your birth month is in January, February or March.

If you have been sent an SMS, just follow the link to book your appointment.

If we do not have an appointment that suits your needs, please call us as soon as possible to get an appointment booked.

What if I don't have a mobile?

We will continue to contact patients without a mobile number via landline.

Where we do not have your mobile or landline we will contact you via post.

Cardiovascular/NDH Clinic

For patients diagnosed with any of the following conditions:

Atrial Fibrillation (AF)

Chronic Heart Disease (CHD)

Chronic Kidney Disease (CKD)

Hypertension 

Non-Diabetic Hyperglycaemia 

Peripheral Aterial Disease (PAD) 

Stroke/TIAs 

 

Diabetic Clinic For patients diagnosed with Diabetes, and any of the conditions listed in the Cardiovascular/NDH Clinic. 
Respiratory Clinic

For patients diagnosed with Asthma or COPD

If you also have conditions listed in Cardiovascular/NDH you will need to attend both clinics.

Dove Medical Practice prioritises the protection of our vulnerable patients, particularly as services transition to digital solutions. For those who are vulnerable, such as our frail patients, you can always call us on our telephone number, and our reception staff will book this appointment on your behalf. 

We encourage patients who have difficulties accessing the internet to reach out to us via telephone. If contacting us by phone is challenging, you're welcome to visit our reception in person. However, we do kindly ask that queues to speak to reception are not formed at 8am as this causes significant backlogs to our telephone lines.

Cervical Smears

If you have been invited for your 3 yearly, or annual smear please contact the surgery via telephone to book. 

We will continue to contact you until you attend for this appointment, or you inform NHS England you would like to no longer be recalled. 

Childhood Immunisations

If you have been contacted to book your child's immunisations please contact the surgery via telephone to book as soon as possible. 

We will continue to contact you until you attend for this appointment, or inform us you would like to formally decline your child's vaccinations. 

Without record of this decline, the surgery and NHS England will continue to contact you. 

NHS Health Checks

We now offer NHS Health Checks for patients who meet our eligibility criteria. 

You will be contacted via SMS or telephone to arrange these appointments.
